1 The prizes are only available to SCSSC members only who submit this form by midnight on Sunday Sunday 5th February 2012 (winners will be notified shortly afterwards).

2 For family memberships, only one entry per household.

3 Winners may bring a guest to the match (member or non member)

4 Travel to match not included.

5 Additional tickets are available at £150.00 each, first come first served.
